**Any student who wishes to participate on any school sponsored athletic team must have the following documents completed before the start of the respective season**

1.    Medical Exam - Please be sure that the Physician provides his/her stamped signature on the document.  Without the stamp the document will not be accepted.

2.    Medical Update  Must be completed prior to the start of every season.  This form should be completed and signed by a parent or guardian.

3.    Parental Permission Form - to be completed prior to the start of each season signed by the parent or guardian.
